Admissions
All potential applicants will be assessed for suitability and appropriateness for treatment at this level of care as defined by the OASAS Level of Care for Alcohol and Drug Treatment Referral (LOCADTR) tool, and Hospitality House’s pre-screening process. We can assist any person not meeting criteria for treatment here with information or a referral to a appropriate agency.
Referring agencies should provide updated information including:
- Signed release of information
- Bio-psycho-social assessment
- Mental Health evaluation (if applicable)
- Health and physical exams
- Lab work (CBC, Micro-urinalysis, TB result)
- Medication list
- LOCADTR determination
- Copy of photo ID
Hospitality House will consider admission for men 18 years and older, regardless of:
- Sexual orientation
- Felony or misdemeanor status- self referred or referred through legal entities
- Health status if the person can physically attend programming
- HIV/AIDS/Hep-C status
- Co-occurring mental health issues
Payments for stay:
Hospitality House accepts the following Medicaid Managed Care Plans:
- Fidelis
- CDPHP
- MVP
- Excellus BCBS
- United Health Optum
Clients may be eligible for public assistance if:
- Over 21 years of age
- Possess less than $2,000 in assets (e.g., automobile, real estate, retirement funds.)
- NYS resident
The following paperwork is requested for all admissions (if you do not have any of the following upon admission, we will assist you in obtaining them):
- Photo ID: Driver’s license or non-driver ID.
- Social Security card
- Birth Certificate
- Insurance Card
